Balloon Mitral Valvotomy (BMV) is a highly specialized, minimally invasive procedure used to treat a narrowed mitral valve a condition known as mitral stenosis. In most cases, this narrowing is a long-term complication of rheumatic fever, which causes the valve’s flaps (leaflets) to thicken, become stiff, and fuse together over time. When the mitral valve cannot open fully, blood backs up into the lungs, leading to severe breathlessness, fatigue, and a racing heartbeat. During a BMV, a specialized balloon is used to gently crack open the fused leaflets, instantly restoring healthy blood flow from the upper to the lower chamber of the heart without the need for traditional open-heart surgery.

A BMV is performed in an advanced cardiac cath lab under local anesthesia and mild sedation, meaning you will remain awake but completely relaxed. A small puncture is made in a major vein in the groin (the femoral vein).
Through this access point, a thin, flexible catheter is navigated up to the right upper chamber of the heart using continuous X-ray imaging (fluoroscopy) and ultrasound guidance. Because the mitral valve is located on the left side of the heart, the cardiologist performs a precise technique called a “transseptal puncture” to gently pass the catheter through the thin wall separating the right and left upper chambers.
Once on the left side, a specialized, highly durable balloon (often called an Inoue balloon) is carefully positioned precisely across the narrowed mitral valve. The balloon is briefly and firmly inflated. Its unique shape applies precise pressure to the edges of the valve, snapping apart the fused leaflets and stretching the opening. The balloon is immediately deflated and removed. The procedure typically takes 1 to 2 hours, and you will notice an immediate improvement in your breathing.
