How Is Stress Produced During SPECT?

SPECT often compares the heart at rest with the heart under stress. If the patient can exercise adequately, treadmill or bicycle exercise may be used while ECG, heart rate, blood pressure, and symptoms are monitored. The tracer is injected at the appropriate level of exercise, and imaging is performed afterward.

Patients who cannot exercise adequately may undergo pharmacological stress. A medicine is given through an IV to produce changes in coronary blood flow suitable for stress imaging. The exact medicine depends on the protocol, patient history, and medical suitability.

How Should You Prepare?

Preparation varies depending on whether the study uses exercise or pharmacological stress. Patients are commonly advised to avoid caffeine for a specified period because caffeine can interfere with some stress medicines. Coffee, tea, cola, energy drinks, chocolate, and some medicines can contain caffeine, so follow the instructions from the nuclear medicine or cardiology team.

You may also receive fasting instructions. Tell the team about prescription medicines, inhalers, diabetes treatment, pregnancy, breastfeeding, asthma, and previous reactions to medications. Do not stop heart or diabetes medicines on your own; the team should tell you what to take on the day of the test.

Wear comfortable clothing and suitable shoes if exercise is planned, and carry relevant previous cardiac reports.

What Happens During the Test?

An IV line is placed in the arm for the radiotracer and, when required, stress medication. ECG electrodes are attached to the chest and blood pressure is monitored.

After the tracer is injected, there is usually a waiting period that allows it to distribute through the heart muscle. You then lie on an imaging table while the SPECT camera acquires images from different angles. Remaining still is important because movement can reduce image quality.

For stress imaging, exercise or medication is used while the clinical team monitors symptoms, ECG, heart rate, and blood pressure. Another set of images is then obtained.

Understanding Your SPECT Results

The cardiologist or nuclear medicine specialist compares tracer distribution in different parts of the heart and, when rest and stress images are available, compares the two conditions. An area with good uptake at rest but reduced uptake during stress is often described as a reversible perfusion defect and may suggest inducible ischemia.

An area with reduced uptake during both rest and stress may represent scar from a previous heart attack, although image artifacts and other factors also need to be considered. ECG-gated SPECT can also provide information about left ventricular pumping function and wall motion.

An abnormal result does not automatically mean a stent or bypass operation is required. The significance depends on the size, severity, and location of the abnormality, symptoms, heart function, coronary anatomy, and overall cardiovascular risk. Further testing such as CT Coronary Angiography or invasive coronary angiography may be recommended when appropriate.

Safety and Limitations

SPECT uses a radioactive tracer and therefore involves a small amount of ionising radiation. Nuclear cardiology laboratories use protocols designed to obtain the required diagnostic information while limiting exposure. Patients who are pregnant, may be pregnant, or are breastfeeding should inform the medical team before the test because special precautions may be required.

The tracer is generally well tolerated. Pharmacological stress can temporarily cause flushing, breathlessness, headache, dizziness, nausea, or chest awareness. Significant complications are uncommon, which is why the test is medically monitored.
