There is no single best test for every patient with chest pain. The correct investigation depends on whether symptoms are acute or stable, the resting ECG, age, cardiovascular risk, ability to exercise, previous heart disease, kidney function, and findings from the initial examination.

A Treadmill Stress Test may be useful in selected patients who can exercise and have an interpretable resting ECG. Stress echocardiography or SPECT can provide additional information about how the heart responds to stress. CT Coronary Angiography provides anatomical images of coronary plaque and narrowing and may be useful in appropriately selected patients. Patients with high-risk or acute findings may require urgent hospital evaluation and sometimes invasive coronary angiography.

Understanding Your Results

A normal result on one investigation does not always rule out every cause of chest pain. For example, a normal resting ECG does not exclude all coronary disease, and a normal echocardiogram does not directly show most coronary blockages. Similarly, an abnormal stress test may require further imaging before treatment decisions are made.
